TITLE:              Social Worker (LMSW)

SUPERVISOR:  Director of Clinic Services

Salary:              $61,808.24/yr

 

Job Summary

The Social Worker (LMSW/LCSW) is responsible to provide social work services to meet the consumer’s needs.

 

RESPONSIBILITIES:

E-            Connotes Essential Functions

E-1          Assesses consumers and completes appropriate sections of the Comprehensive Functional Assessment for consumers annually.

E-2          Completes Initial Psychosocial Assessment and Comprehensive Psychosocial history as needed.

E-3          Develops, writes, implements and reviews social work interventions as necessary to meet the consumer’s needs.

E-4          Develops and provides in-service training for staff in regards to social work and social work interventions.

E-5          Supervises social work assistants in regards to responsibilities by reviewing and signing off on work as required.

E-6          Provides outreach to consumers, family members and/or other people with whom the consumer is involved.

E-7          Coordinates with Accounting office efforts in regards to consumer financial resources.

8-            Participates as a member of the Interdisciplinary Treatment Team.

9-            Participates in consumer admission, movement and discharge procedures as needed.

10-          Participates in projects and committees as assigned.

11-          Attends and participates in Clinic and department meetings as necessary.

12-          Provides consultation for staff when requested in regards to suggestions, ideas and information as to more effective means of dealing with consumers.

13-          Interacts with consumers, clinical staff and other agency staff in a positive manner to facilitate the development of good working relationships and to enhance consumer relationships and their skill development.

14-          Liaison between the consumer/resident/DT and those community resources utilized by the consumers such as day programs, workshops, hospitals, etc.

15-          Ensures required information is maintained in both day habilitation and residential consumer records.

16-          Assists family members in obtaining legal guardianship when appropriate.

17-          Enforces and adheres to all safety policies and procedures.

18-          All other duties as assigned.

 

REQUIREMENTS

  1. Master’s degree in Social Work
  2. Social Worker License
  3. One year experience working or training in the field of Human Services
  4. Good oral and written communication skills
  5. Good interpersonal skills
  6. Valid NYS driver’s license
