TITLE:              LPN/RM or RN/RM

SUPERVISOR:  ADON / ARD

SALARY:           LPN/RM - $39.27/hr or RN/RM - $47.50/hr *EXEMPT

 

JOB SUMMARY:

The RN/LPN RM is responsible for the day to day operations of the residence and to ensure the individuals are receiving the highest quality of care. RM directly provides, oversees and coordinates services and serves as a member of an interdisciplinary treatment team that works to ensure a cohesive and comprehensive plan of care is maintained.

JOB RESPONSIBILITIES:

  • Connotes all essential functions in conjunction with the ADON/ARD and in accordance with IGHL policy and procedure to ensure all needs of the individuals, medical and residential, are addressed timely and thoroughly
  • Creates a positive image of the programs and acts professionally and respectfully on behalf of the individuals in all settings to assure fair and appropriate consideration. Develops positive relationships with individuals and families, being available to address concerns as needed
  • Implements IGHL policies and procedures, trains staff regarding these policies and procedures initially, annually and as needed
  • Conducts assigned fire drills as outlined according to regulatory guidelines
  • Provides 24-hour on-call emergency coverage for site and staffing needs. Will be on call 3 days one week and 4 days the following week, alternating with second site manager for full week coverage. Will flex to ensure staffing is maintained according to critical limits identified for the site with direction from the ADON / ARD as needed
  • Participates in the interviewing, hiring, training, and evaluation of all site staff. Assess’ staff performance and issues re-training and corrective actions as indicated with input from the ADON/ ARD
  • Utilizes appropriate on call procedures for nursing and administration
  • Completes incident reports and investigations relating to issues pertaining to the consumers such as injury, mistreatment, illness, etc.
  • Regularly evaluates the needs and deficiencies of the site / staff in conjunction with IGHL administration and policy
  • Functions as a team leader and, if qualified, QIDP for all individuals residing at the site and leads all interdisciplinary team meetings to ensure individuals needs are identified, presented and addressed, including but not limited to residential, day program/school and medical. Participates in the admission screening process as necessary
  • Oversees site staff, periodically meets with and counsels staff relating to the program, personnel or training needs, completes performance appraisals as indicated and develops yearly objectives to assist staff in becoming a high valued member of the treatment team. Notifies ARD of any employee change in status to ensure all IGHL business forms are maintained.
  • Participates in the development of site-based policies and procedures (including PONS) including implementation and training of all staff initially, annually and as needed
  • Maintains appropriate documentation of staff related issues and personnel records according to IGHL policy. Responds to all program and work-related questions / grievances from staff
  • Ensures staff remain in compliance with state regulations and monitors quality of food preparation and sanitary conditions to kitchen to ensure a safe, clean environment. Monitor weekly food purchases. Coordinates the preparation and modification of consumer specific diets.
  • Completes and reviews all required paperwork specific to life plans / CFA’s to ensure the individuals needs are being identified and met as appropriate. Oversees consumer developmental plans
  • Responsible and accountable for maintaining approved budgets, making recommendations as needed
  • Responsible and accountable for accurate and up to date individual records both residential and medical
  • Ensures billing is complete and accurate
  • Ensures all medical appts are scheduled, completed and followed up on appropriately, with all associated medical documentation for these appts implemented and maintained
  • Ensures that all individuals needs are reviewed as per program and IGHL regulations
  • Supervise AMAP staff as applicable. Notifies RN/ADON of AMAP retraining or recertification needs
  • Works with treatment team to identify and maintain placement in appropriate day program / school setting. Coordinates transportation of individuals to / from day program, work, school, recreational or social programs, etc.
  • Ensure individuals are living in a safe and hygienic manner, acts as a resource for infection control and universal precaution use to all staff. Assists and supervises maintenance and/or housekeeping staff to ensure that all IGHL policies and regulations regarding environment standards are maintained
  • Reviews medication supply and storage. Ensure all needed medications and supplies are available
  • In the event of a medical emergency, assist in seeking emergency assistance, arranging medical transportation and ensures appropriate medical documentation is available. Arranges follow up care and coordination of services as related
  • Coordinates admission and discharge process for hospitalizations of individuals as per IGHL policy
  • Maintains accurate and up to date medical records, care plans, interventions, plans of nursing service (PONS) and outcomes. Coordinates with site ADON as needed. Adheres to confidentiality and privacy regulations in accordance with healthcare policies and IGHL requirements
  • Communicates openly and regularly with administration, nursing leadership and all other members of the interdisciplinary treatment team who provide services for the individuals
  • Ensures all site equipment is functional, available, and up to date. Notifies appropriate parties for any needed repairs or restocking
  • Communicates with the pharmacy as needed to ensure accuracy in orders, billing, etc.
  • Acts in a direct care capacity when needed
  • Adheres to all safety policies and procedures
  • Obtain and maintain all required certifications through IGHL
  • All other incidental and job-related duties as required and assigned

 

REQUIREMENTS

  1. Valid NYS RN license
  2. Experience with disabled population preferred
  3. Valid NYS driver’s license
  4. Must be able to lift at least 50 lbs
